For forty years, doctors watched patients' disease progress instead of treating it — and deliberately kept them in the dark
In the American Tuskegee experiment, conducted from the 1930s to the 1970s, researchers set out to observe the untreated progression of a disease in Black men who were denied available treatment.
In 1932, the US Public Health Service began a long-term study in Macon County, Alabama, aimed at observing the natural progression of untreated syphilis in several hundred Black men, most of them poor farmers.
Participants were never told the study's real purpose — they were told they were being treated for "bad blood," and in exchange for participating received free meals, transportation, and burial services, but not actual treatment for the disease.
Even after penicillin became a widely available and effective cure for syphilis in the 1940s, researchers deliberately withheld it from participants so as not to disrupt their observation of the disease's progression — the experiment continued until 1972, when it was exposed by the press.
The significance: the Tuskegee case became one of the most shocking examples of research ethics violations in history and directly contributed to the introduction of mandatory informed consent for participants in medical studies in the US.
